Office Policies

Scheduling Appointments

You may schedule an appointment by calling our office Monday-Wednesday 9:00am -5:00pm and Thursday 8:00am -5:00pm and Friday 8:00am-4:00pm.  Or you may schedule online through Zocdoc or through our patient portal.

Canceling Appointments

We have a 24 hour appointment cancellation policy.  Please call us as soon as possible to cancel your appointment.  This will allow us to offer your appointment time to another patient. And you will avoid the cancellation fee.

Late Appointments

We respectfully request that you make every effort to be on time for your appointment.  We strive to stay on schedule so that you do not have a long wait time. If you arrive late for your appointment, we will try to fit you in.  However in the event that we cannot fit you in on the same day, we will gladly reschedule for the soonest available appointment.

Emergencies

We offer same-day appointments for patients experiencing urgent medical problems. Please call our office and our staff will gladly accommodate emergency cases approved by our physician.

Payments

Co-payments are due at time service is rendered. We kindly request that new patients complete a credit card authorization form, which may be used to cover balances of charges not paid or covered by your insurance carrier.

Insurance Information

Insurance coverage will vary according to your plan.  Patients are responsible for the knowledge of their own benefits.

Labs & Results

Laboratory results are checked daily and we provide our patients with easy access to their lab results via the patient portal. Results are available after they are reviewed by your physician. Please be advised NO NEWS IS GOOD NEWS, if an abnormal result is reported we will contact you by phone as soon as we receive it.

*Please allow 2 weeks for Pap Smears, Pathology and Biopsy results.

 Prescription Refills

All medication refill requests should be submitted several days before finishing meds.

Please allow 2-3 business days for us to process you refills request. You may call our office to request a refill or submit request via the patient portal. You may also ask your pharmacist to submit a request on your behalf.

*If it has been more than one year since your last appointment, please make an appointment to have your medication refilled.

Medical Records

You are entitled to copies of your medical records.  You must fill out and sign a medical release authorization form.  This form may be faxed, mailed or physically dropped off to our office. Limited fees may be charged –reasonable, cost-based fee for labor for copying or creating summary or explanation. If applicable, cost of supplies and postage.
